Our Intake Documents are listed below. Please print and complete the applicable forms and bring them with you to your first session (do not email completed forms). For a couple or family intake sessions, each should complete the applicable documents.
If you are unable to print and complete these at home, please arrive at your first session at least 15 minutes early.
Adult Intake Form: required for all intake sessions (individual, couples, Anger Management, and Drug & Alcohol)
Adolescent Intake Form: required for all intake sessions (individual, couples, Anger Management, and Drug & Alcohol)
Information for Clients: required for all individual, couples, and Anger Management counseling intake sessions
Beck’s Depression Inventory: required for general and couples intake sessions
Beck’s Anxiety Inventory: required for general and couples intake sessions
Parental Consent to Treatment for Adolescents: required for children under 14 years old
Confidentiality Agreement: required for all minors whose parents are divorced/divorcing, unless parent signing has sole legal custody
Clients paying with insurance should arrive a few minutes earlier and bring their insurance card.
